a. Unzip the files to a location you desire.
b. Open "options.php" and change all the options to your liking.
c. Upload "formsend.php," "form_page.php," and "options.php" to the same
   directory.
d. Use "form_page.php" to send messages to the email address specified.

You can style the form on "form_page.php" however you like using either CSS or
(X)HTML.
